) disciplinary record of previous school
) proof of residential address (only a municipal account )
) Salary advice for both parents and/or guardians and persons responsible for account

Consent and Indemnity

I hereby give my consent for my child to take part in extramural activities for the school, including educational excursions / tours, cultural and sports activities while attending this school.

I fully understand and accept that my child’s participation in all tours, excursions and sports activities shall be undertaken at indemnity, hold harmless and absolve the Governing Body, Principal and Staff against any or all claims whatsoever that may arise in connection with my aforesaid child in the course of any such practice, tour / excursion or cultural or sports activity.

Toestemming en Vrywaring

Ek gee hiermee toestemming dat my kind, solank hy/sy aan hierdie skool verbonde is, mag deelneem aan die skool se buitemuurse aktiwiteite, insluitende opvoedkundige uitstappies / toere, sport– en kulturele aktiwiteite.

Ek begryp ten volle en aanvaar dat my kind op eie risiko deelneem aan alle oefeninge, toere, uitstappies en sportaktiwiteite en ek onderneem om die Beheerliggaam, die Hoof en Personeel namens my, my eksekuteure en trustees, my eggenoot / eggenote en my kind te vrywaar van enige eise wat mag ontstaan as gevolg van die verlies of beskadiging van enige eiendom van bogenoemde kind. Bogenoemdes word ook gevrywaar van eise weens

beserings van die genoemde kind tydens so ‘n toer / uitstappie, sport– of kulturele aktiwiteit.

Acknowledgement of debt

According to Article 39 of the South African Schools’ Law, all parties who commit themselves to this form are liable to pay the compulsory school fees. According to Article 40 of the South African Schools’ Law the school may enforce payment of these compulsory school fees. The parties to this application undertake to pay all legal fees, including lawyer and reclamation fees, if the school should have to take legal steps to recover school fees.

We agree that both parents, father and mother, and/or guardian, are jointly and severally liable for the school fees as determined by statutory regulation annually, and that fees will be paid in accordance with the requirements set out from time to time by the school for the duration of our/my child’s school career at Harrismith High School.

Erkenning van skuld

Kragtens Artikel 39 van die Suid-Afrikaanse Skolewet, is alle partye wat hulle tot hierdie vorm verbind aanspreeklik om verpligte skoolgelde te betaal. Kragtens Artikel 40 van die Suid-Afrikaanse Skolewet, mag die skool betaling van hierdie verpligte gelde afdwing. Die partye tot hierdie aansoek onderneem om alle regskoste te betaal, insluitende prokureurs/kliëntfooie en invorderingsgelde wat deur die skool aangegaan is ingeval die skool geregtelike stappe moet doen om skoolgelde te verhaal.

Ons stem saam dat beide ouers, pa en ma, en/of voog, gesamentlik en afsonderlik verantwoordelik is vir skoolgeld soos jaarliks bepaal deur statutêre regulasie en dat skoolgeld betaal sal word soos van tyd tot tyd uiteengesit word deur die skool, vir die duur van ons/my kind se skoolloopbaan te Hoërskool Harrismith.

Mandate to gather personal information

We understand the school’s right to seek confidential information regarding personal finances and income and agree to the above when application for financial relief is applied for.

Volmag tot die invordering van persoonlike inligting

Ons verstaan die skool se reg om vertroulike inligting insake persoonlike finansies en inkomste te ondersoek wanneer daar om finansiële verligting aansoek gedoen word en gee hiermee ons toestemming daartoe.

General

Herewith we apply to enrol the above-mentioned child as learner of Harrismith High School and declare that he/she complies with the basic requirements.

We undertake to abide by the rules and disciplinary code of the school, as well as possible amendments thereto.

We understand and confirm that the principal or class teacher will act on behalf of the parent/guardian in any circumstance and during any time we/us entrust our child to the care of the school.

We understand that, although any reasonable attempt will be made to avoid loss or damage to the learner’s clothes or property, the school cannot be held accountable for such losses or damages.

We undertake to reimburse the school for any damage to school property caused by the learner.

We give consent that our child may be photographed, videoed or interviewed at school events, and that this may be released and reproduced for the use in media, newspapers, internet and any other form of social media

Algemeen

Ons doen hiermee aansoek om die kind van wie die naam op hierdie vorm verskyn as leerder by Hoërskool Harrismith in te skryf en verklaar dat hy/sy aan die basiese vereistes voldoen.

Ons onderneem om by die reëls en dissiplinêre kode van die skool te hou sowel as by die onderskeie veranderings in die reëls en kode van die skool te hou sowel as by die onderskeie veranderings in die reëls en dissiplinêre kode wat van tyd tot tyd gemaak mag word.

Ons begryp en bevestig dat die hoof of enige sodanige gemagtigde persoon in die plek van die ouer/voog sal optree in enige omstandighede en gedurende enige tyd wat ek/ons ons kind aan die sorg van die skool toevertrou het.

Ons begryp dat, alhoewel elke redelike poging aangewend sal word om verlies en skade aan leerders se klere en besittings te verhoed, die skool nie aanspreeklik daarvoor gehou kan word nie.

Ons onderneem om die skool te vergoed vir enige skade aan skooleiendom wat deur ons kind veroorsaak is.

Ons gee ook toestemming dat ons kind gefotografeer, gevideo of ‘n onderhoud mee gevoer kan word tydens enige skool byeenkoms, en dat dit gebruik kan word in die media, koerante, internet en alle sosialemedia platforms.

Domicillium citandi et executandi

It is agreed by the parties that their respective addresses, herein before set out on this application form, shall serve as their respective Domicillium citandi et executandi viz the addresses to which all notices, reports and/or invoices regarding school fees may be sent.

Daar word ooreengekom dat die adres aangegee op hierdie aansoekvorm sal dien as die aangewese Domicillium citandi et executandi en dus die adres waarna alle kennisgewings of enige ander dokumente versend mag word deur enige van die partye met betrekking tot verslae en/of skoolgelde.
